INSURANCE

I am “in network” (INN) with Aetna. If you would like to use Aetna health insurance, please let me know upfront so we can do a benefits check and ensure that I am in network with your specific Aetna plan.

An important note about using insurance: It is your responsibility to know and understand your mental health benefits, including any co-pays, co-insurances, or deductibles.

By using insurance you agree that:

  • You have verified your benefits, cost, and coverage.

  • You have notified me of any/all health insurance coverage that you have (primary insurance, secondary insurance, Medicare, etc).

  • As a courtesy, I will run a benefits check however I can cannot guarantee that I will be given correct information from the insurance company.

  • Should there be any discrepancies, any owed amount is your responsibility.

  • As with any insurance coverage, estimates provided are non-binding and subject to change once claims have been processed.

Out of Network Benefits (OON)

Some insurance companies will reimburse patients directly for the cost of some or all “out of network” services if patients submit a “superbill” (which I can provide).

If I am not in network with your insurance and you are interested in seeking reimbursement, I recommend checking your out-of-network benefits first.

Lyra EAP

I am contracted with Lyra Health, an employee assistance program (EAP) specializing in mental health benefits. If you work for a company that uses Lyra for EAP benefits, your company may pay for a certain number of sessions each year. I am not contracted with any other EAP programs.


FEES AND RATES

  • Intake Session (50 minutes): $240

  • Therapy Session (45 minutes): $180

  • Extended Therapy Session (55 minutes): $200

  • Missed appointments or cancellations within 48 hours: Full session fee.

  • Payment in full is required at the time of service. Credit, debit, HSA/FSA cards are accepted. No cash or checks.


SCHEDULING AND AVAILABILITY POLICIES

  • I have a firm 48-hour cancellation policy. If you cancel at hour 49, you will not be charged. If you cancel at hour 48, you will.

  • Commitment Policy: For me to block out a spot on my schedule for someone, they to be using it at least 90% of the time. If you are someone who might need frequent or last minute schedule changes, I would not be a good fit.

  • I do not provide crisis management services, same day appointments, or emergency phone sessions. I am not available “on call” or for after-hour emergencies. If this is a service that you have used in the past or think you may need in the future, then this practice would not be a good fit.
